1. Clinical pharmacy

• Upon admission to the emergency department, systematically query home medication and adjust it in the electronic medical record, so that home medication continues correctly upon admission and the correct adjustments can be made.
• Monitoring of possible drug interactions upon admission.
• Tips to support optimal drug treatment.
• Responsibility for an efficient electronic prescribing system, which also alerts prescribers and nurses of relevant drug interactions.

2. Stewardship

• Training of health personnel.
• Academic detailing: using the software system to check the treatment level of selected patients (e.g. with the GDMT score) and then provide targeted feedback to prescribers.
• Participate in a hospital directive and also promote and disseminate it.

3. Delivery of certain medications such as:

• Tafamidis (Vyndaqel) 61 mg orally.
  • Read: specific cardiomyopathies – ATTR cardiac amyloidosis.
  • Certification to be completed electronically by the cardiologist via CIVARS.
• Iron IV (Injectafer or Monoferric).
  • Read: intravenous iron (iron deficiency).
  • Certification to be completed electronically by the cardiologist via CIVARS.
• Mavacamten (Camzyos) 2.5 mg, 5 mg, 10 mg, 15 mg orally.
  • Read: hypertrophic cardiomyopathy.
  • Certification to be completed electronically by the cardiologist via CIVARS.
